Associate Program Coordinator - Quality Outcomes

Summa Health Medical Group / CPC Plus Program

Summary:
Provides project management support to improve quality outcomes reporting for the Summa Health Medical Group value based care programs.

Works collaboratively with internal and external resources to achieve maximum financial returns yearly.

Completes medical record reviews with a focus on HEDIS and risk adjustment coding gaps information for value-based payers in the ambulatory environment.

Formal Education Required:
a.

Associates degree in a health or social services related field or equivalent combination of education and/or experience

Experience & Training Required:
a.

Two (2) year experience in medical or health services field to include:
i.

Medical record review for HEDIS
ii.

Experience with EPIC Electronic Medical Record
iii.

Project Management
b.

Knowledge of NCQA, HEDIS, CMS and Health Plan standards and delegation requirements as they relate to quality management.

Other Skills, Competencies and Qualifications:
a.

Able to navigate complex situations independently and complete tasks with minimum direction.
b.

Able to set priorities and manage workload to maximize outcomes.
c.

Proficient in Microsoft Office programs.
d.

Maintain confidentiality and HIPAA standards use appropriate documentation requirements.
e.

Knowledge of Social Determinants of Health for reporting purposes.

Level of Physical Demands:
Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently.

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials.
