Job description

Position Summary

:

The Medication for Opioid Use Disorder (MOUD) Care Coordinator serves an essential role in supporting the MOUD team’s implementation process, providing case management services and goal setting directly with patients.

The MOUD Care Coordinator will provide individual and group patient treatment, and link patients with necessary community resources.

This person will provide comprehensive quality services to patients working collaboratively with staff to provide team-based coordinated care.

The MOUD Care Coordinator will have contact with patients and will be in constant contact with the providers and members of the MOUD team in order to communicate patient's needs and shape patient care.

This person will also manage administrative duties and billing needs for the MOUD team.

  • Duties:

    Patient Care Coordination

  • Triage patients appropriately, providing case management and care coordination, including assistance with tracking medication management and treatment monitoring through the electronic health record system (EPIC).
  • Insurance authorization and troubleshooting for MOUD medications.
  • Patient linkage to additional community support (drug treatment services and mental health care referrals as needed) and tracking referrals.
  • Relationship building and facilitation of ancillary services (including patient transportation).
  • Assist with obtaining release of information from patient to better facilitate coordination.
  • Community Referrals

  • Assist the prescribing physician in making referrals to community providers to provide referral resources for those services ACHD cannot provide directly.

    Provide insight into needs related to patient’s community resources available to them.
  • Tracking & Reporting

  • Record, track, and report on pertinent data and use outcomes to drive system changes that improve care over time.

    Ensure all documentation is in place for DEA inspection.

    Track feedback from patients on patient experience—from patient assessment, patient satisfaction, and patient counseling.
  • Patient Eligibility, Assessment and Enrollment

  • Perform screenings, assess eligibility and coordinate patient enrollment into MOUD program.
  • Patient Education

  • Create materials for patient education on how buprenorphine treatment is delivered, what patients can expect to experience at each stage of treatment, and community and caregiver resources for maintaining treatment.
  • Provider Meeting & Communication

  • Manage and organize weekly MOUD meetings.
  • Assists in public health related emergencies.

  • Performs related work as required.
