EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 120 impactful programs across Long Island and NYC, is looking for a dedicated full time Case Manager to join our Sanctions Intervention Program.


Wage: $23.69 per hourSchedule: 8:30 to 4:30pm ET Monday through FridayLocation: DSS South West 2 S 2nd Street in Deer Park with frequent travel to Coram, Riverhead, and Smithtown.


Primary Purpose of Job:

The Case Manager is responsible for interviews of individuals on sanction status with the Suffolk County Department of Social Services (DSS), for data collection and compilation regarding these individuals and their families, and for referrals to Department of Labor and DSS case management for services.


Principal Duties & Responsibilities:

+ Conduct interviews of DSS-sanctioned clients.



+ Complete assessments on clients.

Collect and compile data regarding clients and their families.



+ Make referrals to Dept.

of Labor for clients that demonstrate employability.



+ Refer clients to EMP (Employment Management Program) at DSS for case management services when indicated.



+ Assist with entering assessment information into the database.



+ Provide targeted interventions, which may include making appropriate referrals, and arranging for mental health services, child care, housing, etc.



+ Provide Supervisor with transmittal information including any recommendations indicated for follow-up actions by DSS to assist families in moving from sanction status.



+ Travel to all program locations as needed including; Deer Park, Coram, Smithtown, Riverhead, and Hauppauge

+ Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.


Knowledge, Skills, & Abilities Required:

+ High School Diploma or GED educational equivalent, a degree in social or behavioral sciences preferred.



+ Minimum of one year of work experience in human services required.



+ Must have good communication skills, both oral and written.



+ Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.



+ Must have access to personal automobile and possess a valid New York State drivers license.
