Job description

Job Summary



The case worker participates as a member of the health care team by contributing to the understanding of the social, emotional and economic factors, which relate to the patient’s illness, disability and age.

Investigates, evaluates and acts upon high risk and referred cases and provides direct casework to patients and families.

Provides counseling, crises management, bereavement services and arranges for home care, utilizing available community resources and arranges for extended care facility placement as indicated.

Maintains open communication with all members of the interdisciplinary health care team and community agencies to facilitate care within the continuum.

The case worker focuses on timely; appropriate discharge plans and coordinates with the case management department nurses to ensure benefits for services and insurance authorization.
Education
Minimum:
Bachelor Degree in Social Work, Counseling or other related health care field

Experience
Minimum:
Clinical rotation through educational program.
Preferred:
One year experience in a hospital setting.
Other Requirements

Excellent communication skills and skills in customer service.

Commitment to the delivery of qualitypatient care with the ability to develop and maintain effective, professional relationships with all members of the health care team.
