**Job Description:**

The Community Health Worker (CHW) is accountable for partnering with clinical and health plan teams to engage, advocate, educate, navigate, and coordinate with—and on behalf of—individuals identified as having social needs that impact their health.

The CHW may visit patients and members in home or community settings as needed, while also collaborating with care management and care teams (e.g., clinics, emergency departments, home care, health plans, etc.).

Typically, a CHW works under the direction of an ambulatory Care Manager and/or a clinic RN or MA.

**Essential Functions**

+ Builds trust with patients and their families to engage them in their health and empower them to recognize their potential and create community connections.
+ Applies motivational interviewing and health coaching techniques during patient interactions.
+ Ensures timely enrollment in, and referrals to, health and community resources, and assists patients in navigating these services.
+ Maintains regular contact with patients through phone calls, digital communications, home visits, and visits to community settings.
+ Collaborates with care managers and clinical teams to deliver coordinated, comprehensive care that addresses social needs as part of the patient's care plan.
+ Collaborates with Community Health to support partnerships and maintain relationships with key community stakeholders to improve health and social outcomes.
+ Communicates empathetically and clearly with patients and caregivers, honoring their preferences and ensuring understanding.
+ Contributes to and maintains a comprehensive knowledge base of community resources and their functions.
+ Participates in professional development and networking opportunities to enhance skills and knowledge.
+ Utilizes strong organizational skills to manage tasks and appointments, and applies critical thinking to resolve complex issues and navigate community resources.

**Minimum Qualifications**

+ Demonstrated communication skills, including listening, speaking, and writing.
+ Ability to work independently as well as collaboratively in and in group settings.
+ Proactive mindset with the ability to take initiative .
+ Willingness and ability to conduct home visits.
+ Availability to be present on-site in clinical and community settings.
+ Basic computer proficiency .
+ Current driver’s license, insurance, reliable transportation, and an acceptable driving record.
+ Must obtain Community Health Worker (CHW) Certification within 18 months of hire into role.

**Preferred Qualifications**

+ Community Health Worker (CHW) Certification
+ Experience working with diverse populations, including individuals with limited financial resources, educational backgrounds, and communication barriers.

+ Ongoing need for employee to see and read information, labels, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
+ Frequent interactions with providers, colleagues, customers, patients/clients and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
+ Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy.

This includes frequent computer use for typing, accessing needed information, etc.
+ For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
