As the Community Resource Navigator , you will play a pivotal role in individuals' lives, making a significant impact through:
+ Receive and respond to phone calls, messages, and online referrals from individuals and families in need of assistance, inclusive of referrals from other community service providers
+ Completes intakes, initial assessments, referrals and education, advocacy, budgeting, screening, and provides material goods and financial assistance and documents finds disbursed as appropriate
+ Assess client needs and assist with negotiating systems to obtain entitlement services as needed to enhance self-sufficiency
+ Assists in the promotion of Catholic Charities through the attendance of community events, speaking opportunities, serving on local committees as requested by supervisor
+ Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver using the approved ACH screening.

Community Resource Navigator Maximum Salary: $20.00
Community Resource Navigator Schedule: Monday - Friday 8:30 -4:00
Qualifications:
+ High School Diploma or GED
+ With 3-5 years of experience
+ Associate's Degree in Human Services or a related field
+ With 1-3 years of experience
+ Experience working with a diverse population
