Job description

Summary:


Under the general direction of the Director of Cancer Services and Ambulatory Care and within hospital and department policies and procedures abstracts information form the medical records of patients being accessioned into the computerized Oncology Database.

Enters information into a computerized database (IMPAC) system to establish an Oncology Data Management data bank from which to draw information for annual reports site specific studies data requests and assist physicians with short and long-term cancer patient evaluations etc.

Responsibilities:
Reviews the medical records of patients to ascertain demographic patient identification data medical history and diagnosis information such as method of diagnosis primary site extent of disease and treatment information such as types of systemic therapy and agents utilized.



Retrieves Patient Disease Index and reviews against the active and suspense database contained in the IMPAC to ascertain if a patient is a case for follow-up newly diagnosed and requires accessioning into the database or has developed a subsequent new primary (requires new accessioning into the database) requires a staging form to be appended to the medical record and/or completed/signed by the attending physician etc..



Reviews the entire medical record to ascertain the following: patient identification data; history of admissions; method of diagnosis; history and types of treatments; and intent of treatment (cure or palliation) against the criteria for cancer case selection set forth by the American College of Surgeons (AcoS) the Rhode Island Department of Health The Miriam Hospital Cancer Control Committee and other regulatory agencies.



Abstracts information on newly diagnosed patients directly into the computerized database (IMPAC) from medical records including patient identification and socio-demographic data primary site and staging of disease modes of treatment types/sites of recurrence and survival status.

Interprets data and uses the American Joint Committee on Cancer Manual for recording extent of primary tumor and absence or presence of distant metastases ICD-O (International Classification of Disease for Oncology) for tumor nomenclature and coding Roads Manual for coding and the Rhode Island Census Tract Coding Guide for assignment of Census Tract Codes.



Receives pathology reports and reviews both clinical history and microscopic diagnosis to identify new patients and update information in the appropriate electronic database (IMPAC).



Receive changes from other source lists from the Hospital Association of Rhode Island which are reviewed against the electronic databases for completeness and accuracy of data.

Discrepancies are investigated before any changes are made to the electronic record and death information is entered into the proper fields.

Monitors and records lifetime follow-up on each patient listed in the registry via phone contacts letters to physicians patients and other agencies as appropriate.



Compares daily obituaries in the newspaper and on the Internet against master file of registered and suspected cases.

Notes date of death in appropriate electronic record.



Ensures staging form is in the record if appropriate.

If staging form is not in the record places appropriate staging form in the record and refers the record to the incomplete section if applicable or Manager HIS for completion by the physician.



Meets minimum productivity standards for abstracting records of one per hour.

Maintains 95% accuracy rating.

Maintains 90% follow-up rate.



Participates in weekly Tumor Board meetings.

Record recommendation(s) and enters it onto the patient abstract located in the computerized IMPAC database.

Maintains attendance log.



Participates in quarterly Cancer Control Committee meetings.

Responsible for accurately recording and disseminating minutes.

Maintains attendance log.

Other information:
BASIC KNOWLEDGE

Working knowledge of medical terminology anatomy and physiology.

Knowledge of Cancer Registry data collection requirements of the American College of Surgeons National Cancer Registrars Association federal state local governmental agencies and other regulatory agencies.

Knowledge of computer software programs i.e. IMPATH Microsoft Office Windows Operating System etc..

Demonstrates competent computer and word processing skills.

Maintains continuing education credits in the field of Cancer Registry Data Management.

Maintains credentials i.e. CTR RHIA RHIT etc.



EXPERIENCE:

College graduate (minimum requirement associate degree) preferred.

Certified Tumor Registrar (CTR) or CTR eligible.

In lieu of degree successful completion of an Allied Health Information Technology program Cancer Registry Management program and/or a minimum of five years of data management experience in an approved AcoS Cancer Registry.

Experience in coding abstracting and staging of cancer data using ICD-O Coding TNM and AJCC Staging Systems.



WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Works with physicians and other clinical disciplines in a research setting.

Applies computer and database management knowledge for formulating ad hoc queries from the various databases as requested by physicians and/or other clinical disciplines.

Works in normal office environment.

Sits stands and walks intermittently throughout the day.

May lift up to ten pounds.
