Dialysis Vascular Access Program Coordinator (RN license required) - 20 hours per week
Overview:
Massachusetts General Hospital Dialysis Vascular Program is a multidisciplinary program that includes Nephrology, Interventional Radiology, and Transplant Surgery.
The goal of this program is to facilitate access care in patients with chronic kidney disease.
The Dialysis Vascular Access Program Coordinator serves as patient liaison and coordinate patients’ access care among the Interventional Nephrology Center, dialysis facilities, nephrologists, nurse practitioners and surgeon on all aspects of vascular access related issues.
As the central contact for ensuring smooth flow of patients’ care, the coordinator will ensure the quality and safety of patient’s access needs.
The coordinator is responsible for collecting patients’ clinical information, evaluate patients’ clinical situation, and schedule surgery/interventional radiology appointments, and relay information back to the patients and their outpatient dialysis unit.
Access coordinator is also responsible for entering dialysis access related patient data into the electronic database and maintaining accurate records of all access events.
This data will be used to create various reports for quality assurance, process improvement, and financial performance purposes.
As the central person for the program, the coordinator will be responsible for maintaining open communication and relationships with outside dialysis facilities.
This will include relaying information from our clinicians to the referring facilities about patient status.
Also, as the central communicator, the coordinator will maintain patient lists with status updates to ensure that urgent patients are streamlined and that less urgent patients are not overlooked.
The employee will be working for the Nephrology Division and Interventional Radiology under the supervision of Nephrology Administrative Director and the Medical Director for Interventional Nephrology.
