Responsibilities

The Health Program Coordinator I will coordinate Diabetes Program deliverables.

Responsibilities include managing contracts; providing technical assistance to organizations achieving, maintaining and sustaining National Diabetes Prevention Program (National DPP) and diabetes self-management education and support program (DSMES) recognition and accreditation; supporting subcontractors’ implementation of National DPP and DSMES programs in healthcare and community-based settings; developing and enhancing state and local partnerships for delivery of National DPP and DSMES programs; contributing to the development of subcontractor workplans and budgets; providing guidance for National DPP and DSMES Medicare and Medicaid billing and provider enrollment; facilitating monthly internal/external project team calls; contributing to evaluation and program narrative reports; developing program specific presentations and website content; and related activities.

Other appropriate duties as assigned.

Minimum Qualifications

Bachelor’s degree in a related field and three years of experience in program coordination in a public health, health, health regulatory, or human services related program; OR an associate’s degree in a related field and five years of such experience; OR seven years of such experience.

At least one year of experience must have included supervision of staff and/or program management.

A master’s degree in a related field may substitute for one year of such experience.

Preferred Qualifications

Graduate degree in public health, public/health care administration, social work, nursing, or related field.

Experience in the development, delivery, coordination or implementation of evidence-based diabetes prevention and/or self-management programs in community-based organizations, health systems, state or local health departments, and/or federally qualified health centers.

Experience in the management of state or federally funded contracts.

Familiarity with Medicaid and Medicare policy and billing processes for the National Diabetes Prevention Program and/or diabetes self-management education and support programs.

Experience coordinating diabetes public health initiatives with community pharmacies and/or community health workers.
