ABOUT A PATH OF CARE HOSPICE

A Path of Care Hospice is a partnership of home health, hospice, and private home services agencies.

At A Path of Care Hospice, we aim to relieve pain, improve comfort, and help our patients take charge of their lives.

We work directly with their doctors to develop individualized care plans to ensure our patients receive the best care possible to heal.

In every interaction with our customers, we demonstrate our core values of credibility, accountability, responsiveness, and empathy.

We strive to gain the trust of our patients by working hard, assuming responsibility for our actions, and bringing sunshine to their lives.

Always eager to help, you provide a variety of caregiving and support services, including running errands like grocery shopping or driving them to appointments and doing occasional light housekeeping and laundry.

You form genuine connections with patients as you offer them companionship by sitting with them at their bedsides, helping them write letters, and carrying out their care plans.

You also make sure that their homes are safe environments, reducing the likelihood of accidents and promoting a timely response to illness.

You are meticulous about reporting any changes or concerns promptly.

In this way, you provide real peace of mind for them and their families.
