POSITION SUMMARY:


Responsible for the accurate flow and processing of referrals for home health, palliative care, and hospice care.

Responsible for the overall management of the intake department including performance management and process improvement.


EDUCATION/CERTIFICATION:



  • Graduate of an NLN-accredited school of nursing; BSN preferred.

  • Possession of a current license to practice as a registered nurse in the State of Connecticut.


EXPERIENCE:



  • Two (2) years’ full-time clinical experience, at least one of which was in a home care agency.


COMPETENCIES:



  • Knowledge of Agency policies and principles.


  • Capable of implementing Agency’s goals and directions.


  • Knowledge of communities served.


  • Competent in clinical health principles.


  • Knowledge of state and federal regulations and accreditation standards.


  • Ability to work positively with staff: peers, subordinates, and superiors.


  • Ability to function independently, within scope of responsibility.

  1. Oversees the processes of the Intake Department:

    1. Ensures that referrals are accepted on a timely basis.

    2. Documents referrals and NTUs on shared board.

    3. Assures that the highest quality and professionalism is provided to all customers.

    4. Oversees day to day function of Intake nurses.



  2. Responsible for identifying and obtaining all necessary information for referrals:

    1. Assess W10 and all documentation from referral sources for appropriateness for home care services.

    2. Obtains verbal order for SOC that includes a description of the patient’s condition and the services to be provided by the Agency.

    3. Assigns accurate ICD-10 codes to referral diagnosis.

    4. Obtains history and physical and other documentation required for coding.

    5. Coordinate all disciplines/services for successful initial assessment.

    6. Obtains all necessary documentation for face-to-face document.



  3. Responsible for establishing and maintaining positive relationships with referral sources within the hospital, physician offices, or healthcare facilities.

    1. Information resource for home health services.

    2. Assist complex patient conferencing as needed.

    3. Resource for early intervention, need for cooperative relationships.

    4. Serves as a liaison between referral sources.

    5. Assist with payor information and verification process.

    6. Performs other duties as assigned.



  4. Responsible for overseeing and managing the performance of the referral nurses in the department.
