We are currently seeking a LPN/RN for 2 nd shift.
Our team provides compassionate care to elderly people who need assistance in managing their health care needs.
We provide several levels of care from independent living, to nursing care, skilled nursing care and memory care.
**We are offering a $1500 sign on bonus to the candidate who accepts this position!**
**Responsibilities:**
+ Follow the nursing model to promote holistic care for the residents and a team approach for staff
+ Direct the daily activities of the nurses, CNAs, and CMAs in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures
+ Participate in the admission, discharge and transfers of residents as required.
+ Assess changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly
+ Transcribe physician orders to medical records and carry out orders as written
+ Prepare and administer medications per physicians’ orders and observe for adverse effects
+ Perform wound treatments per physicians’ orders, observes for changes and documents accordingly as indicated (RN only)
+ Schedule follow up appointments for residents
+ Provide nursing leadership to nursing team members assigned to the unit
+ Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc.
in accordance with current policies and procedures
+ Perform rounds to ensure resident needs are being met
+ Collaborate with other members of the interdisciplinary team to ensure residents’ needs are holistically met
+ Participate in management team meetings to discuss census changes, resident changes in status or care need, or concerns
+ Assist in evaluating work performance of nursing personnel and implement discipline according to operational policies and as directed
+ Inform appropriate departments of concerns or complaints and follow up as needed
+ Report any incidents or unusual occurrences to the director of nursing and participate in the investigative process as needed
+ Participate in end-of-life care as required
+ Ensure that there is an adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs
+ Initiate, review and update care plans as required
+ Participate in the survey process when inspection surveys occur
+ Ensure that infection control principles are followed and assist in correcting deficiencies in practice
**Requirements:**
+ Current unrestricted Iowa LPN or RN nursing license
+ CPR certification
+ Schedule availability to work every third weekend and every other holiday
+ Pre-employment physical and TB test required
+ Excellent interpersonal skills, including customer service, patience, empathy, and compassion
+ Experience working in long term care a plus but on required
**Job Details:**
+ Schedule: 2 nd shift: 2:00-10:00 pm; including working every third weekend and every other holiday
+ Starting wage:
+ LPN: $30.00 per hour; plus pay for related experience
+ RN: $32.00 per hour; plus pay for related experience
+ Shift differential for 2 nd shift: $2.00 per hour
+ Incentive pay for picking up additional hours
