Job description

This position is responsible to plan, organize, develop and direct operations of Westchester Harbour, with specific emphasis for the Memory Care Unit, reporting to the Assisted Living Administrator.

Supervises and coordinates resident care in accordance with resident needs, government requirements, and EveryAge policies.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

•Follows established standard of nursing practices and implement facility policies and procedures.

•Identifies resident needs based on comprehensive assessment.

•Delivers care following physician’s orders, nursing standards and facility policies and procedures.

•Documents care according to regulatory requirements and facility policies and procedures.

•Assures all regulatory standards and requirements are met.

•Supervision over residents in regard to their activities of daily living, including bathing, dressing, grooming, toileting, changing of bed linens, positioning, transfer, mobility, and incontinence care

•Act as the Resident Care Director in their absence

•Assists and teaches unit staff in appropriate resident care, utilization of resources, regulatory compliance, orientation and in-service programs.

•Complete resident assessment paperwork upon admission, and as required by regulation, and as needed

•Performs duties and consistently works cooperatively with administration, all nursing personnel, physicians, community agencies, residents, families and consultants.

•Prepare, assist, and document the administration of medication, following accepted practices, and in accordance with State regulations

•Obtain and maintain proper certification for licensure annually

•Communicate with residents and resident’s families in a polite and professional manner

•Conducts hands on educational presentations for clinical staff.

•Attends and participates in committees, meetings, conferences and educational programs as requested.

•Provides for the health and safety of employees by contributing to the maintenance of a safe environment.

•Performs job duties utilizing safe working methods/procedures and actively participates in the organization’s safety and loss prevention program.

•Assures proper medications, treatment and documentation pursuant to physician’s orders.

•Instructs personnel in new nursing techniques, procedures, and use of equipment.

•Works alongside CNAs and Medication Techs to assure care is provided in accordance with policies and standard nursing practices.

•Works c to assure orders are carried out accurately and timely as prescribed by Practitioner.

•Responds to emergencies as needed.

•Recognizes changes in resident(s) condition and takes appropriate action timely.

•Assists in clinical matters, including in-house assessments with recommendations (based on clinical judgement) regarding resident condition and proper steps to take towards a good clinical outcome.

(call MD, send to ER, initiate standing orders already written, etc)

•Maintains strict confidentiality of patient, employee and company information.

•Good working knowledge of the State regulations, health care regulations, and OSHA regulations

•Clinical Assessments both ON and OFF campus for the purpose of ascertaining appropriateness of admission to facility

•On Call Rotation

Minimal Education:

Graduate of accredited school of nursing.

Current NC Nursing License 

Minimal Experience:

Minimum of two (2) years License Practical Nurse experience in a skilled nursing facility and/or assisted living facility.

One (1) year of supervisory experience preferred.

Other Qualifications:

Current Nursing License

CPR certification

Strong communication, leadership skills and interpersonal relationship skills.

Demonstrated clinical competency and provision of expert care to patients with either predictable or unpredictable
