Marram Health Center, a Federally Qualified Health Center (FQHC), is seeking an Outreach Specialist to provide individual and community orientation as to the availability of programs that assist the uninsured to obtain healthcare coverage.
The Outreach Specialist assists in making appointments for the uninsured with the goal of establishing them in a medical/dental home for all eligible individuals/families.
As directed, the Specialist also participates in events and gives presentations.
This position will be based at our Gary office, with daily outreach and travel mainly to our Hebron and Portage locations. Marram Health Center is an FQHC operated in conjunction with Porter-Starke Services, Inc., a Community Mental Health Center based in Valparaiso, IN.
Starting at $19/hour and up based on experience.

PRIMARY RESPONSIBILITIES
- Contact and build relationships with clients in the target population.
- Assists with Marram Health Clinic enrollment, making appointments as needed and getting to those appointments.
- Advocate for the clients in their interactions with the health care, government and social services systems.
- Help clients acquire the skills that they need to ensure effective access to and use of necessary medical and support services.
- Assist clients in understanding health and other information that they receive.
- Assist in enrollment in Indiana Medicaid including Presumptive Eligibility, coordinating with the Outreach and Enrollment Coordinator.
- Collaborate with other care providers regarding participant needs.
- Provide supportive services necessary to promote health and independence and to support lifestyle changes to improve health and functioning.
- Maintain accurate and complete record documentation and data collection.
- Coordinate and attend community activities that promote the Center.
- Distribute education materials regarding services the center offers.
- Provide preventive health education around substance use and safer sex practices.
- Refer clients to Marram for medical needs.
- Attends staffing’s, clinical supervision, in-services and staff meetings.
- Performs other duties as assigned.
REQUIRED CERTIFICATION/LICENSE
- Indiana Navigator and Certified Application Counselor training, examination, and certification processes must be initiated upon hire and all continuing education and certification requirements are kept current
QUALIFICATIONS
- Previous related experience working in related discipline preferred.
- Experience or training in Federally Qualified Health Center preferred.
- Must be able to maintain a valid driver’s license.
- Must be able to maintain valid automobile insurance.
- Must have own vehicle.
