Job description

Duties Description This position will provide supervision to one or more POS 1s as a part of the Division of Quality Improvement's newly formed CCO/HH (Care Coordination Organization/Health Home) oversight team.

The POS 2 will play an integral role in the completion of designated reviews to assess the CCO/HH programs in OPWDD.

Additionally, the incumbent is expected to review and advise on CCO/HH quality improvement initiatives and progress made from a quality improvement perspective, review and make recommendations on CCO/HH Quality Improvement/Quality Management Plans and specific QI projects.

This position provides management and supervision to staff in a variety of tasks and related work to support CCO/HH programs and reports to the POS 3, CCO/HH Team Director.

The Program Operations Specialist 2's (POS 2) will:

· Be willing to learn and become an expert at all CCO/HH related activities, operations and policy/practices in order to facilitate quality improvement.


· Work independently and as a part of both small and larger teams.


· Assist with and manage review activities and reviewers as assigned by the Director of the CCO/HH Unit.

Will be responsible for a caseload of review tasks and functions.
· Work as a part of the CCO/HH team in the monitoring and evaluation of CCO/HH performance, including annual case reviews to be conducted by the CCO oversight team staff to assess CCO performance.
· Provide monitoring and evaluation of a targeted sample of high needs individuals using the Person Centered Review methodology to be conducted by the CCO oversight team to assess CCO performance.


· Provide assistance in the monitoring and evaluation of at least one annual specially designed focus area to assess CCO/HH performance in a special issue area (., incident management, comprehensive assessment, core functions of health home model, other review topics as requested,
· Participate in the CCO/HH redesignation/recertification reviews in conjunction with the Department of Health, Bureau of Program Certification (BPC) surveyors and other OPWDD staff as applicable.


· Participate in both on and off-site work assignments.
· Will be responsible for the preparation of reports, written assignments, data analysis, identification and summary of needed areas of improvement for CCO/HH programs at both the system and program level.
· Will assist with oversight of review activities and will be responsible to complete a review caseload.
· Assist with the development of training materials, conduct trainings and present information for internal and external audiences, prepare materials for and facilitate meetings and workgroups as needed, be prepared to interface with a wide range of both CCO, Department of Health and OPWDD staff.
· Provide supervision and management to at least one POS 1 and assist with management of team caseload and assignments.
· Assist CCO Team Director with tasks that could include development of processes and guidance, review of policies, responding to complaints, analysis of data on programmatic strengths and challenges and other activities.
· Must be able to prepare reports and written information on the performance of the CCO/HH program as a whole and for each CCO/HH based on individualized review activities.
· Be prepared to learn evidence-based QI tools and methods including data driven approaches, analysis of performance measures, and other related activities as appropriate to the activity.
· Review of plans of corrective action; and provide enhanced oversight if needed/applicable
· Provision of training and technical assistance to CCO/HHs as needed/directed
· Be able to provide written and accurate summaries of assigned CCO/HH performance at aggregate/system level for each CCO/HH and at program level and at the request of DQI or OPWDD leadership;
· Be able to use critical assessment and judgement skills to assess and determine the quality of supports/services delivered for reviews that are not compliance based by using analytical skills, interviewing, review of case records and documentation and other methodologies in a team based environment that crosses OPWDD Divisional boundaries and outside met/not met decision-making approach
· Desire and ability to engage in ongoing continuous quality improvement and/or project management as a team member or team leader as situations dictate and to learn from team members and staff internally, cross divisionally, and cross agency.
· Exhibit the ability to analyze policy and situations to evaluate options and course of action where no structure currently exists; to think creatively and to make sound recommendations and judgement based on evidence presented.
· Show comfort with both written and spoken modes of communication.
· Additionally, as assigned by the Director, the POS 2s will:
· Coordinate and participate in internal/external QI committees;
· Participate in developing/implementing QI projects;
· Internally coordinate with BPC and IMU;
· Coordinate across OPWDD as needed for CCO oversight activities
· Participate in the CCO clinical community of practice, and liaison with DOH.

Minimum Qualifications Eligible List
Candidates must be reachable for appointment on Current Civil Service Eligible List for this title.

OR

Transfer
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law.

The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Reassignment
Current Office for People with Developmental Disabilities employees with one year of permanent, contingent permanent, or 55 B/C service as a Program Operations Specialist 2.

If you have less than one year, you must be currently reachable on the eligible list.

OR

Provisional
The Civil Service promotional examination for this title was held on September 20, 2025.

To be eligible for a provisional appointment, candidates must have one year of permanent competitive or 55-b/55-c service as a Program Operations Specialist 1, Developmental Disabilities Program Specialist 1, Licensed Master Social Worker (and all parenthetics)***, Mental Hygiene Program Evaluation Specialist 2, Resources and Reimbursement Program Development Specialist 1, Standards Compliance Analyst 1 (Developmental Disabilities), or Utilization Review Coordinator, and must have taken this examination.

Candidates who have not passed the examination or do not achieve a reachable score on the resulting eligible list may not be retained in these positions.



***Due to the May 23, 2024, title structure change/ reallocation, permanent competitive, non-competitive NY HELPS*, or 55-b/55-c service as a Licensed Master Social Worker 1 and 2 and all parenthetics is also qualifying.
