Responsibilities

The Registered Nurse (RN) will:
Evaluate and assess the individuals’ health needs and medical/nursing issues Designs and implements the individuals’ treatment plans; train and supervise direct care staff in implementing goals and treatment outcomes Communicate medical and health related concerns with the Community Nurses Maintain all requisite documentation, including all in-services and special instructions to staff: prepare monthly and episodic nursing notes Provide direct medical/nursing services on a regular basis, on emergency basis and as prescribed by Physician Request, administer, record, store, count and log medications; keep logs of destroyed medications Communicate with staff in regard to all of their issues and concerns presented to them Communicate effectively with on site supervisors in regard to participants’ status as well as to relay and discuss all issues and concerns Conduct or assist with ongoing trainings in required health-related topics and specialized training, as needed Perform all other duties, as assigned Qualifications: 2+ years of Nursing experience Graduate of an accredited School of Nursing NYS Registered Nurse (RN) license Solid assessment, clinical, and documentation skills Patient oriented Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized
