Job description

Position Summary

Reporting to the Director of Nursing, the Senior Clinical Case Manager is an integral member of the Columbia Health Medical Services interdisciplinary team.

This role oversees and integrates case management and care coordination functions to ensure the delivery of high-quality, effective, and patient-centered care.

The Senior Clinical Case Manager provides both direct case management to students with complex medical conditions and supervision of case management staff.


Key responsibilities include assessing medical information to determine appropriate levels of care, coordinating services and resources, fostering collaborative relationships with internal and external providers, and ensuring optimal continuity of care for patients with chronic and acute medical conditions.

The Senior Clinical Case Manager also plays an important role in workflow and process improvement, addressing factors that impact health equity and system resources, and facilitate a collaborative approach to patient care across the continuum.


This is an essential onsite role.

Some hybrid work options may be considered.

Responsibilities


Supervision and Leadership (40%)

  • Directly supervise case management staff, providing mentorship, ongoing feedback, and professional development opportunities.

  • Evaluate the performance of direct reports, addressing work performance issues and consulting with the Director of Nursing and Human Resources regarding corrective actions.

  • Approve time-off requests and manage workload distribution to ensure balanced caseloads.

  • Facilitate regular clinical supervision, case consultations, provide feedback regarding case management issues, review, analyze, and monitor case management work for quality assurance issues, recommend crisis intervention methods and direct ongoing process for formal and information assessment strategies to meet the diversity of patient care, needs and determine (re) allocation of resources.

  • Maintain up-to-date and accurate patient records; review clinical documentation for quality and timely completion of case notes.

  • Supervise and develop direct reports in the delivery of effective case management including screening, assessing, evaluating risks, follow-up, implementing, planning and evaluating outcomes.

  • Provide periodic program updates and productivity analysis to senior leadership.
  • Clinical Case Management (30%)

  • Provide comprehensive case management for high-risk students with chronic and acute complex medical conditions.

  • Meet with case management team to review clinical case history for incoming students and initiate follow-up communication with entering students, who have identified as having significant health needs.

  • Gather and evaluate information from relevant sources to determine service needs and refer to appropriate resources.

  • Effectively coordinate care with internal and external providers, and community resources to optimize medically necessary services.

  • Use appropriate screening criteria, knowledge, and clinical judgment, to assess patient’s current functional level and develop and monitor ongoing care plans.
  • Collaborative Partnership (25%)

  • Establish and maintain collaborative relationships with internal departments, external stakeholders, and community agencies to support holistic patient care.

  • Identify at-risk populations and engage in proactive outreach to mitigate the need for crisis management.


  • In partnership with Medical Services leadership, hold primary responsibility for aspects of the Medical Services Quality Assurance (QA) Program.

  • Collaborate with leadership to assist with strategic planning, policy development, and accreditation preparation.

  • Advise Columbia Health leadership on trends in student health needs and recommend strategies or resource allocation to address those needs.

  • Participate in New Student Orientation and other outreach events as assigned.

  • Minimum Qualifications

  • Bachelor’s degree in nursing or social work required.

  • Current New York State RN or LCSW license.

  • Minimum of 4 years related clinical experience and previous supervisory experience.

  • Demonstrated ability to handle high-volume workload, exercise independent judgment, and manage concurrent issues and multiple stakeholders.

  • Strong clinical judgment, problem-solving, decision-making skills.

  • Demonstrated ability to communicate effectively with students, providers, and campus partners.

  • Experience working with diverse populations and individuals who hold multiple, intersecting identities, including but not limited to: historically marginalized and underrepresented populations, various ages, and developmental status, cultural, ethnic, and socio-economic backgrounds, international status, disability, gender, and sexual identities, and presentations.

  • Ability to foster collaborative relationships across departments and with external partners.

  • Proficiency in computer systems and familiarity with electronic medical records (EMR).

  • Understanding of acuity levels within the student population and the ability to adapt case management approaches accordingly.
  • Preferred Qualifications

  • Master’s degree in Nursing, Social Work, or a related field.

  • Previous experience in a hospital, ambulatory care, higher education, or a similar healthcare setting.

  • Certified Case Manager (CCM) credential.
