**Responsibilities:**
GENERAL SUMMARY
The Sr Public Health Project Specialist organizes, coordinates and supports county-wide efforts to improve the health of the community including The Monroe County Community Health Improvement Plan.
This position coordinates a community health needs assessment every three years and manages a community team to implement strategies to address priority needs.
This process is required of non-profit hospitals and the health department in Monroe County, and as such this position assures compliance to national and state regulations for UR Strong and UR Highland, including the involvement of the Center for Community Health & Prevention (CCHP).
This work is done in collaboration with the faculty Director of Community Health Education and Policy.
**ESSENTIAL FUNCTIONS**
+ Planning and coordination of the efforts for interventions identified in the Community Health Improvement Plan (CHIP).
+ This may involve researching best-practices and evidence-based interventions, attending community meetings and forums, meeting with hospital champions to assure implementation, applying for grants and programs relevant to the implementation plan, and evaluation of the outcomes for quality improvement.
+ Coordinate the progress on implementation, track and evaluate the projects, note barriers and successes to the team.
+ Oversight of the creation of the Monroe County Community Health Needs Assessment (CHNA) and the Community Health Improvement Plan (CHIP) every three years in response to federal and state regulations for non-profit hospitals.
+ The CHNA/CHIP requires population data analysis, priority assessment, community input and establishment of objectives and metrics.
+ Report on CHNA/CHIP to hospital leadership teams.
+ Maintains and implements compliance with annual reporting requirements to the state department of health and federal government through tax submissions.
+ Assure dissemination of CHNA/CHIP to the public.
+ Administration and coordination of the meetings and activities of the Community Health Improvement Workgroup (CHIW) which includes representatives from the hospitals, health department and prominent community organizations in collaboration with the Director.
Establish an annual budget, invoice hospitals for resources, and manage the finances of the CHIW.
Oversee a “mini-grant” process to provide funding for exceptional community-based proposals.
+ Report on the progress of the mini-grantees and the CHIW to stakeholders.
+ Provide leadership and content expertise to the Center for Community Health & Prevention (CCHP) for community health needs and emerging topics in community health.
+ Connect CCHP directors to community-based agencies working in topic areas of interest.
Inform CCHP faculty and staff of community initiatives such as the 1115 Waiver, Doula reimbursement changes, etc.
+ Stay informed and involved with important community-based agencies and initiatives.
+ Work with the Director to assist in the UR Strong community benefits reporting annually.

**MINIMUM EDUCATION & EXPERIENCE**
+ Bachelor’s degree with major coursework in appropriate health, social science, public health, or closely related field.

+ Must have independent transportation to various community-based agencies in and around Rochester, NY.
