Job Responsibilities:

Job Summary


The CLTS Waiver Program is one of Wisconsin’s Home and Community Based Services (HCBS) Medicaid Waiver programs, federally authorized under § 1915(c) of the Social Security Act.

HCBS waiver programs provide Medicaid funding that may be used to fund allowable support and services that promote individuals in the waiver access and inclusion in their home and community.

Eligible individuals include youth between the ages of 0-22 with developmental, severe emotional, and/or physical needs, who are also functionally and financially eligible for Medicaid.

Working under the direction of the CLTS program and Milwaukee County Children Youth and Family Services, provide supervision for program staff and manage the provision of all CLTS services at WCS.

This includes providing direct training, coaching, leadership, supervision, and oversight of a team of 8-12 Service and Support Coordinators.

Essential Duties and Responsibilities

  • Be a champion of serving youth and their families experiencing disability needs.

  • Embrace and champion family centered, trauma informed, strength based, culturally intelligent services.
  • Provide supervision, leadership, and role modeling in a strength-based environment to a team of approximately 8-12 Service and Support Coordinators.
  • Attend meetings as requested with the Milwaukee County CYFS Management Team.
  • Keep Milwaukee County CYFS informed of emergency or critical incidents and submit the necessary critical incident paperwork in CITR.
  • Monitor service utilization, review monthly case notes, and provide feedback to Support and Service Coordinators and Milwaukee County CYFS as requested.
  • Notify Milwaukee County CYFS of all staff changes.

  • Maintain accountability for all Milwaukee County CYFS Policies and Procedures.
  • Ensure that accurate family demographic information is maintained by the Support and Service Coordinators and changes reported to Milwaukee County CYFS.
  • Provide orientation and training in Support and Service Coordination processes to all new Support and Service Coordinators, promoting individualized care and use of natural supports.
  • Provide regularly scheduled team meetings to share information regarding Milwaukee County CYFS changes and address areas of concern.
  • Develop, implement, and maintain a call system from Monday through Friday, 8:00 am to 4:30 pm, to ensure families’ access to Support and Service Coordinators.
  • Participate in mandatory trainings and offered in services and ensure that Support and Service Coordinators do the same.
  • Provide coverage for Support and Service Coordinators, as needed.
  • Maintain internal records related to Milwaukee County CYFS youth and their families using recommended chart format dividers.
  • Conduct regular internal audits of SSC case notes, monthly contacts, six-month and annual reviews, and ISP documentation to ensure accuracy, timeliness, and compliance with county/state standards.

    Prepare for external audits (MetaStar, county QA) and implement corrective action plans when needed.
  • Notify Milwaukee County CYFS when a new Support and Service Coordinator has been hired, providing CYFS with the name and date hired, as well as their qualifications and contact information.
  • Notify Milwaukee County CYFS when a Support and Service Coordinator resigns, has a change in name or change in phone or cell phone number.
  • Maintains employee files, which include the Support and Service Coordinator’s training records.

    Milwaukee County CYFS maintains the right to periodically audit agencies to ensure adherence.

  • Job Qualifications:

    Required Qualifications

  • Must possess one of the following combinations of degree/experience: Bachelor’s degree in a relevant area of human services, education, sociology, social work, etc.

    and one year of experience working with specific target group.

    Bachelor’s degree in a non-relevant area of human services and 4 years of experience working with specific target population.

    Associate’s degree with minimum of one year of experience working with specific target group.

  • Knowledge, Skills and Abilities

  • Mental Health/Human Development - Working knowledge of mental health needs, cognitive, developmental, and physical disabilities, and positive child/youth development.
  • Trauma-Informed Care - Patience and understanding of traumatic life situations.
  • De-escalation Skills - interact with escalating youth and/or parents in a calm and professional manner and respond appropriately in crisis situations.
  • Technological Aptitude - Must be comfortable with navigating electronic health records, including in Avatar and utilizing Microsoft Word, Excel, PowerPoint, etc.

  • Time Management Skills - Ability to maintain schedules, meet deadlines and multi-task.

    PHYSICAL DEMANDS:


    Duties require possible activity while monitoring or interacting with youth in the program.

    Staff may be required to use physical intervention in extreme situations.

    WORK ENVIRONMENT:


    Work is performed primarily in a busy office environment managing caseloads of youth, young adults, and families.

    Some of the work is done sitting at desk using a computer; requires outreach in the community and families’ homes; much of the outreach is done in urban neighborhoods and several hours per day may be spent driving; hours average 40 per week; flexible work schedule include some hours outside the normal work schedule on evenings and weekends.
