Job description

Duties and Responsibilities:

  • Performs initial intakes, which includes welcoming families to the program, reviewing policies and procedures with them, and completing psychosocial assessments;
  • Works with adults and children in RRH to develop strengths-based case management plans that are updated at least quarterly and signed by client and case manager - Case plans incorporate goals for both adults and children related to core goal areas: finding and maintaining stable housing, increasing income, and family stabilization and support in areas including mental health care, substance abuse treatment, and parenting skills;
  • Meets regularly with families on caseload to develop case plans and follow-up as needed;
  • Networks with community resources and makes referrals as appropriate for housing, employment, substance abuse treatment, mental health treatment, training, etc.;
  • Collaborates regularly with Youth, Employment, and Wellness Specialists to ensure that specialist support is available to address specific needs as part of the family’s case management plan;
  • Provides administration, supervision, and oversight to team of Housing Stability case managers and/or youth specialists to ensure that team functions on a day-to-day basis to meet all the needs of families assigned to that team, as well as all reporting responsibilities;
  • Provides regular individual supervision to team members to address immediate client needs and to ensure clinically-sound practice from a strengths-based perspective;
  • Participates actively in clinical supervision from Senior Director to ensure that client goals are met and services are provided in a clinically appropriate and ethical manner;
  • Makes referrals for health services to Community of Hope’s clinic as needed;
  • Works with development office on fund raising needs and responds to requests for information on a timely basis;
  • Minimum Qualifications:
  • Minimum Bachelors’ degree required; Master’s degree in social work or related field preferred; professional human services licensure preferred.

  • BSW and MSW degree holders must have a LSWA/LGSW or appropriate social work license.
  • Minimum three years’ work experience in social work or related field.
  • Experience working with families, homelessness, and/or substance abuse strongly preferred.
  • Knowledge of community resources or the ability to become knowledgeable required.
  • Strong interpersonal skills required.
  • Strong organizational skills as well as strong written and verbal skills.
