Duties & Responsibilities: (Essential Functions of Position, Major Responsibilities, Summary of Key Duties, Special Assignments, Relationships) Responsible for planning the delivery and coordination of transition services provided to high school age students and young adults with disabilities.

The coordinator will provide comprehensive services/instruction for youth in the following areas: personal counseling (includes career), transition assessments, diagnostic services, instruction, job development, and placement activities; development of apprenticeships and community based work experiences; compilation of labor market information; collaboration with Iowa Vocational Rehabilitation Services, school personnel and adult service providers; develop transitions plans in coordination of IEP’s (as appropriate) and Employment Plans; teaching of competencies as based on individual needs; maintenance of case records and reports and provide follow-up until the individual reaches age 25.

Student Focus:

+ Oversee students who are interested in additional education after high school by applying to schools, completing financial aid paperwork, buying books and studying for tests.

+ Create transition plans for students with disabilities whether they have an IEP or a 504 plan.

+ Implement individual transition plans for post-secondary careers.

+ Plan and provide vocational counseling as necessary to ensure understanding of disability(ies) and assess and develop appropriate vocational choices.

+ Teach students how to disclose their disability to employers and post-secondary schools.

+ Facilitate job placement by evaluating a student’s job readiness, developing and executing a plan for job seeking activities, contacting employers, providing job analysis and modification/accommodation recommendations, selective placement and follow-up.

Instructing and Assessing:

+ Develop and provide services to assess, prescribe, evaluate and guide high school age students' learning experiences in conjunction with goals and objectives to obtain competitive employment.

+ Develop instruction in the classroom or work field based on individual need.

+ Conduct or oversee assessments to determine student vocational aptitude, interests, or general ability for career areas.



+ Plan and implement vocational instruction and employment information, training in community and independent living skills.

+ Communicate and correctly apply federal and state statutes and rules to colleagues, service providers, employers, and the general public to ensure that legal and appropriate services are provided.



Supervisory Duties:

+ Supervise and follow-up on students placed in the community.

+ Oversees and manages the TAP contract.

+ Creates and submits quarterly reports to IVRS documenting services provided to students, performance measures required by IVRS, budget status of program, business engagement/partnership and follow along/up services.

+ Supervises day to day program, will develop and support job training of youth, and assign TAP Work Liaison to individual students.



Management and Coaching Focus:

+ Initiate and carry out all expected duties including intake of student paperwork, screening, evaluation, goal development, casework activities, placement, guidance and referral.

+ Contributes to IEP meetings, providing results of transition assessment data and updates on student performance at the workplace.

+ Prepares and maintains records and reports as directed by deadlines required by the District, IVRS, AEA, Community Agencies, County and the State.

+ Plans and delivers professional staff development as needed to staff.

+ Work independently and make decisions regarding the TAP program.



Community/Employer Outreach Relations:

+ Work with the business community to learn more about business needs, labor market trends, and future needs of the community.

+ Work cohesively with Iowa Vocational Rehabilitation Services.

+ Develop and complete “Job Development” with employers to establish a partnership for job training sites for youth to learn and work within integrated settings.

+ Understands and develops relationships with youth/families and others representing a wide range of socio-economic, diverse and cultural backgrounds.

+ Collaborate and consults with: Staff, Principals, Teachers, Counselors, Mississippi Bend AEA Staff, Transition Specialist, Parents, Students and Community Agencies.

+ Have a valid driver’s license

+ BA/BS in Education or a Human Services related area OR

+ Degree in social work or relevant profession that emphasizes services to individuals or commensurate experience within the above listed fields.

+ Previous experience working with individuals with disabilities.

+ Communicate effectively both orally and in writing and instruct persons with disabilities, employers, school personnel, and outside support.

+ Self-motivated, flexible and able to meet deadlines.

Must be able to move from one duty or activity to the next independently.

+ Must have computer skills necessary to perform job searches, assist students with on-line and vocational and educational activities on the computer.

Additional Duties: Have the capacity to work in a position that periodically requires flexible hours.
